隨著國家基本公共衛生服務的正式實施，慢性病患者健康管理就是其中一項重要項目。國家明確要求社區衛生服務機構為轄區內確診的高血壓或2型糖尿病患者每年免費提供4次面對面隨訪服務和1次較全面的健康檢查服務。我院為規范慢病隨訪，引進公衛隨訪包。通過手機就能進行隨訪，無需紙質版。大大節省了鄉醫隨訪時間，并且真實性有了很大的提升。

　　With the formal implementation of basic public health services in the country, the health management of chronic disease patients is one of the important projects. The state clearly requires community health service institutions to provide four face-to-face follow-up services and one comprehensive health examination service for patients with hypertension or type 2 diabetes diagnosed in their jurisdiction for free every year. Our hospital has introduced a public health follow-up package to standardize chronic disease follow-up. Follow up can be conducted through mobile phones without the need for a physical copy. Greatly saving the follow-up time of rural doctors and greatly improving the authenticity.

　　培訓后，我院公衛人員積極學習，認真思考，并且下村實際操作，學會操作后幫助鄉醫解決一些上傳失敗的問題。對于一些鄉醫操作還有些不熟悉的情況，我院院長積極安排公衛人員，下村為鄉醫講解疑惑并在現場為其實際操作。?

　　After the training, our hospital's public health personnel actively studied, thought seriously, and went to the village to practice. After learning how to operate, they helped the township doctors solve some upload failure problems.
